Kindly download the nomination form of Malaysia Spa and Wellness Awards 2009 or contact the secretariat for further information.

Nomination Fee
The fixed Nomination Fee to cover administrative expenses is set at RM 200 per nomination per category. Multiple nominations are welcome, but each nomination should be accompanied by a Nomination Fee of RM 200.  

Nomination Deadline
All nominations must reach the Organiser’s office by 20 December 2009.  

Upon completion of the Nomination Form, send the Form with all supporting documentations and RM 200 Nomination Fee per category nominated to:
